MCMC Anticoagulation Clinic

The MCMC Anticoagulation Clinic is managed by clinical pharmacists who work closely with your provider. Although blood thinners are an important part of a long-term care plan for many patients, they require careful monitoring. Our state-of-the-art blood test uses a small finger stick to measure how fast your blood clots and your Anticoagulation Pharmacist will adjust your dose as needed to keep you in therapeutic range.

If you are taking warfarin, please ask your MCMC provider for a referral to the MCMC Anticoagulation Clinic, located at the Celilo Infusion Center at 1800 E. 19th Street, The Dalles.

Medication Therapy Management

Pharmacist review of medication lists across the primary care and specialty clinic settings to support treatment plans and monitor the medication safety and efficacy. Reconciling patient medication lists and ensuring patients have the desired outcomes.
